Please fill out this form so we can customize your Cyber ID tag. You need to choose at least two different colors. If you select the same color twice, we will use different shades of that color.

For the mix ratio, enter it like 1/1. This means we will use equal parts of each color – 50% of color 1 and 50% of color

Some examples:

• 1/1 – Half one color, half another
• 1/2 – One third of one color, two thirds of the other
